Forms

You may know that SGS Outdoors no longer stocks forms.  Most forms can be downloaded from the Girlguiding UK website.  Additionally, we are making commonly used forms available via the the Guide Office and OAC.

Packs of Multipart forms will be sold at the prices listed in Guiding Essentials. They are available as follows:

Office and OAC
  • A/A              Application: holiday/camp
  • A/S              Notification: short camp/holiday/sleepover
  • W/W&A        Application: Warrant/Appointment cards
  • W/R             Volunteer Registration: general
  • W/In            Volunteer Registration: instruction

Office only

  • R/U              Unit Registration
Single copy forms, including those not available electronically, will be photocopied and sold by the Office and OAC at 1p per copy. These are listed below: 
  • T/C              Transfer Card
  • G/O             Information and Consent: one day activity
  • G/C             Information and consent: holiday/camp
  • G/H             Information: general health
  • W/Ren         Guider Renewal of Appointment
ll the above forms will be available for sale at County Day and some other County Events. For forms not on these lists, please apply to the Office.  Should you require forms on days that the office is not staffed, please let us know and we can make arrangements for collection and payment.
